Home Modifications are specific modifications, adaptations, or improvements in an individual’s existing home setting, which, based on the individual’s medical condition, are necessary to ensure the health, welfare, and safety of the individual and enable the individual to function with greater independence in the home to prevent institutionalization of the individual.

Home Modifications, adaptations, or improvements may include but are not limited to the following:

Modifying bathrooms
Modifying kitchens
Installing or building handicap ramps
Installing grab-bars and other durable medical equipment as part of a larger Home Modification project
Widening doorways
Installing specialized electric and plumbing systems necessary to accommodate medically necessary equipment and supplies.
